Delivery of a Budesonide Suspension Formulation from Mesh vs Jet Nebulizers under Simulated Pediatric and Adult Breathing Patterns

Summary

The drug delivery characteristics of nebulized budesonide suspension has been shown to vary considerably depending on the model of nebulizer used. We compared the in vitro aerosolization and drug delivery characteristics of budesonide suspension (Pulmicort, 0.25 mg/mL) from three production equivalent InnoSpire Go (ISGo) mesh nebulizers with those of three breath-enhanced jet nebulizers (LC Plus) when delivering into simulations of adult (Vt=500 mL, f=15 bpm, I:E ratio=1:1) and pediatric (Vt=150 mL, f=25 bpm, I:E ratio=1:2) breathing. Particle size was measured using a NGI with a constant flow of 15 L/min. Delivered dose was assessed using drug collected onto a filter placed before the breathing simulator. End of nebulization was automatic for mesh, and sputter + 60s for jet nebulizers. Jet nebulizers were tapped for 15s after the onset of sputter. Each nebulizer was tested in triplicate. Mass median aerodynamic diameter, fine particle fraction (<5 µm) and GSD for the nebulizers were: ISGo; 4.84µm, 52.1%, 1.71: LC Plus; 6.47µm, 33.1%, 1.85. For breathing tests the mean delivered dose, residual dose and nebulization times for (pediatric) and [adult] were: ISGo; (0.079mg, 0.317mg, 3.78min) and [0.095mg, 0.276mg, 3.57min]: LC Plus; (0.060mg, 0.356mg, 4.78min) and [0.095mg, 0.364mg, 3.79min]. Delivered dose expressed as a percentage of loaded dose for pediatric and adult patterns, respectively were: ISGo; 16.2%, 19.0%: LC Plus; 12.0%, 19.0%. The dose of budesonide delivered by both types of nebulizer into pediatric and adult simulated breathing was within the range of delivered doses found by Berg and Picard (4-20% of loaded dose) when delivering budesonide into simulated pediatric breathing using a range of jet nebulizer systems (Respir Care 2009;54(12):1671-1678).
